Send this application with 3 photos and fee of $16.00 (sixteen dollars) per dog to "AMW Kennel Club and Registry" at the above address, and we will issue registration in your name with a Certificate of Merit mailed directly to the owner of dog.

** Your fee will be returned to you if our judges challenge the application or photo evidence, and adjudge the dog to be of "doubtful breeding"

Puppies must be at least 6 months old for Registration of Merit
